The American Academy of Family Physicians (AAFP) approved the conference for up to 11 prescribed 'live' continuing education credits (CME). Providers should claim only the credits commensurate with their attendance at each session.

CME credit request form is available online and at the registration desk. Certificates of completion will be issued shortly after the conference.

1:15 PM–2:00 PM Rural Health Care Leadership: Cultivating the Soil for People to Grow Benjamin Anderson, President and Chief Executive Officer, and Samuel Akidiva, MD, Chief Medical Officer at Hutchinson Regional Healthcare System, will walk us through how Rural America continues to face significant challenges and deepening disparities in access and health outcomes and how we might look at these long-term, daunting issues through an environmental lens. With communities in mind, what if we liken our family, friends, neighbors, and teammates to seedlings that require a healthy environment at work and home to grow and thrive? This session will introduce practical tools to improving community wellness that are rooted in improvement science and leadership development. Designed around USDA’s Four Principles of Soil Health, these interventions are already yielding fruit.

2:00 PM–3:00 PM Healthy Skepticism: An Antidote for Misinformation Lindsey Leininger, Ph.D., Faculty Director, Center for Health Care, Clinical Professor of Business Administration at The Tuck School of Business at Dartmouth, will share a constructive approach to apply healthy skepticism to scientific and healthcare claims. She will highlight the unique challenges of “unsettled” medical science. Dr. Leininger will highlight the factors that contribute to misinformation within and external to the healthcare system in our post-COVID world. She will offer insights into practical ways and resources for communities and primary care practices to address misinformation.

3:30 PM–4:30 PM AI in Primary Care: Insights, Opportunities and Experiences. Amelia Sattler MD, Clinical Associate Professor and Associate Program Director of the Stanford Healthcare AI Applied Research Team, Stanford School of Medicine, will present specific examples to help us envision how AI can improve clinical workflows.

4:30 PM–5:30 PM Value-Based Care: Reflections as a PCP, Policymaker, Purchaser and Payor. Alice Chen, MD, MPH, Centene Corporation’s Chief Health Officer, will share insights on value-based care for primary care, drawing upon her experience as a primary care physician, policymaker, health system leader, Executive Forum co-chair for CMMI’s Health Care Payment Learning and Action Network, and role as Chief Health Officer at Centene.

8:30 AM–9:30 AM Rebuilding Primary Care—from Fragments to Continuity Ilana Yurkiewicz, MD, Clinical Assistant Professor at Stanford University School of Medicine, Co-Medical Director of Primary Care for Cancer Survivorship, and author of the book Fragmented: A Doctor’s Quest to Piece Together American Health Care, will share insights into how primary care is deteriorating “one fragment at a time,” highlighting key problems including disorganized technology, non-communicating care teams, and uncompensated time leading to a “lose-lose” transaction model of care delivery for patients and providers. Dr. Yurkiewicz will propose how we can work, both as systems and individuals, to address the root causes of fragmented care and move towards the ideal of whole-person primary care grounded in continuity.

9:30 AM–10:30 AM Preserving Kidney Health in Rural Hawai‘i Kalani Lukela Raphael, MD, University of Utah (JABSOM Alum), Professor of Internal Medicine, will provide practical suggestions for primary care providers in rural Hawai‘i to preserve kidney health in people with type 2 diabetes, hypertension, and other risk factors for chronic kidney disease progression.
